Surgical management of left main stem stenosis: Current outcome and consideration of contemporary percutaneous therapies.

The left main coronary artery disease has poor natural history. Sudden death, massive myocardial infarction, angina and poor quality of life indicate the ominous nature of the disease. Coronary artery bypass grafting remains the ultimate choice of its management. Recently the improved safety of stent implantation gained popularity as a reasonable alternative to bypass grafting. The current outcome of surgical management as a land mark for competing therapies is not sufficiently available in literature. During the period from November 1997 to July 2001, 38 patients underwent coronary artery bypass surgery for left main stem stenosis at Federal Postgraduate Medical Institute, Shaikh Zayed Hospital, Lahore. There was one non cardiac death 3 months after surgery. Four patients had recurrence of angina. Thirty three patients were asymptomatic and majority were able to return to their jobs. The immediate and long term results of surgery were good.
