Breast milk and specialised feeding in preterm infants.

Milk feeds should begin as soon as possible within one week after birth in preterm infants even if only subnutritional quantities are given. The milk of choice is preterm human milk from the infant`s own mother. If not use immediately, expressed breast milk should be refrigerated at 3-4C and this can be stored for up to 2-3 days before use. Supplementation with human milk fortifier should begin when full enteral feeding is achieved and this is generally continued until breast feeding is established or hospital discharge. If the mother`s milk supply is temporary interrupted or if she decides against expressing breast milk for her own infant, donor human milk can be used, even though the nutritional inadquency of term milk is a major disadvantage. Alternatively, preterm formula can be used, starting at an energy content of 67kcal/100ml and when full enteral feeding is achieved, the energy content is increased to the recommended 80 kcal/100 ml. Preterm formula is generally continued until hospital discharge when a standard formula is used. This is a review article.
