Clinical importance of Creatine Kinase, Troponin-T and Myoglobin determination in patients with acute chest pain.

Background: To compare different cardiac markers in patients suffering from acute chest pain. Method: Thirty-five patients, 29 males and 6 females aged 30-75 years were selected. They presented with chest pain of 1-12 hours duration. Myocardial infarction (MI) was diagnosed on the basis of clinical history, physical examination and electrocardiogram (ECG). Blood samples were drawn and serum creatine kinase (CK), CK-MB, myoglobin and troponin-T were determined. Results: Statistically significant differences (p<0.001) were noted in the mean ± SD concentration of both CKMB and myoglobin when comparing the patients presenting within 1-3 and 4-9 hours after the onset of chest pain. In contrast, non-significant difference (p<0.05) was noted in the mean ± SD concentration of troponin in the above mentioned patients. Conclusions: It is proposed that a sensitive and specific cardiac marker like myoglobin should be advised to assist the clinician for the rapid triage of patients to suitable management at presentation and subsequently.
