Carcinoma Hypopharynx - A pragmatic approach in management.

OBJECTIVE: The aim of this study was to rationally manage diagnosed cases of carcinoma hypopharynx according to the stage of the disease and to compare the results of different modalities of treatment. DESIGN: Prospective, comparative. PLACE AND DURATION OF STUDY: Department of ENT and Head and Neck Surgery Combined Military Hospital Rawalpindi from January 1998 to June 2001. PATIENTS AND METHODS: A total of 27 patients being managed for carcinoma hypopharynx were included in this study. The diagnosis was confirmed by biopsy and histopathological examination. Clinical staging of all the patients was done on TNM criteria so as to offer appropriate treatment modality. The treatment modalities were as follows; radiotherapy alone, pre-operative radiotherapy, post operative radiotherapy, chemo radiotherapy and palliation. The outcome of various modalities was recorded. RESULTS: Radiotherapy alone yielded the best results. The results of surgery followed by radiotherapy were satisfactory while surgery following radiotherapy had dismal results. Chemo radiotherapy was not tolerated well by the patients and produced even worse results than in the group receiving palliative treatment. CONCLUSION: The study concluded that attention to the clinical staging of the disease and proper choice of the modality of treatment after assessing the general health of the patient will have an impact on the post treatment quality of life. In advanced disease the effects of palliative measures are often more merciful than the added morbidity and discomfort of a treatment option.
