Dissecting Aortic Aneurysm with Myocardial Ischemia: presentation and management in acute emergency.

A 19 year old, diagnosed case of SLE with grade III aortic regurgitation, presented with acute chest pain and left ventricular failure. His ECG was suggestive of ST depression in II, III, V3 to V6. His echocardiography revealed severe wall motion abnormalities in both ventricles, a dissecting flap in close proximity to the right coronary ostium. Clinical diagnosis of Acute Aortic Dissection was made and patient was scheduled for emergency surgery next morning. This case report describes the management of this patient during pre-operative, operative and post-operative periods. He was successfully discharged home on 11th post-operative day.
