Presentation, Progression and Prognosis of Stroke in the Hypertensive, the Diabetic and the Normotensive Normoglycemic.

Introduction: Stroke remains a leading cause of death world-wide. The incidence has shown a general decline in some western countries over the last three decades. This is probably due to increased awareness of risk factors, improvement in prophylaxis and better surveillance of those with a propensity to develop it. A previous stroke makes an individual more susceptible to further such episodes. Risk factors include hypertension, diabetes mellitus, hyperlipidemia, cigarette smoking, cardiac disease, drug abuse, heavy alcohol consumption, a family history of strokes and AIDS. The purpose of this study was to gauge the effect of hypertension and diabetes mellitus in patients presenting with strokes and compare it with normotensive and normoglycemic individuals who developed the same condition. Study: Forty five patients presenting with Stroke were studied over a two year period. They were divided into 3 groups, each comprising 15 patients. Those suffering from hypertension and diabetes mellitus were included in Group A, those with hypertension alone in Group B while the normotensive normoglycemic individuals were placed in Group C. Group A patients had the earliest onset, highest mortality and maximum complication rate while those in Group C had the best overall prognosis. We conclude hypertension and diabetes mellitus are potent risk factors for stroke, the effect being augmented when both act in concert, while the normotensive normoglycemic fare best of all.
