Term Prelabor rupture of membranes conservative vs active management.

Objective was to compare the outcome of conservative and an active policy in management of prelabor rupture of membranes at term. Conservative policy was to wait for 24 hours for spontaneous onset of labor and then induce with prostaglandin (PGE2). Active policy comprised of immediate induction with PGE2 vaginal pessary or with oxytocin. Study was carried out at Services Hospital from January 2000 to April 2001. Ninety-four patients were studied who were randomly allocated either to immediate induction or conservative management groups. Group I: 48 patients managed conservatively. Group 11: 46 patients managed actively. Parameters studied were caesarean section rate, rate of forceps delivery, spontaneous delivery, length of labour, pyrexia in labor or peurperium. Results showed fewer caesarean section 8.3% Vs 14.6%, less forceps delivery 10.4% Vs 17.3% and more spontaneous deliveries in group I. There was no difference in length of labour or pyrexia in labour in both groups. The above results favor conservative management.
