Disseminated Tuberculosis of Brain.

A 45 year olf man presented with one month history of fever, headache, vomiting, diplopia, dysphagia, dysarthria and ataxia of gait. Neurological examination revealed multiple cranial nerve involvement, ataxic spastic quadreparesis and signs of meningeal irritation. His ESR was 110 mm in 1st hour. Cerebrospinal fluid showed a high protein level (280 mg/dl), low glucose (20 mg/dl) and high white cell count (240 /cc with 98% lymphocytes). MRI Brain revealed extensive tuberculomas and tubercular abscesses scattered throughout both cerebral hemispheres and brainstem. Patient improved dramtically on anti-tuberculous treatment.
