Prognostic value of Prostate Specific Antigen (PSA) in Stage D Carcinoma of the Prostate after Bilateral Orchiectomy.

Thirty patients with stage D prostatic carcinoma were evaluated to see the prognostic value of prostate specific antigen PSA. Bilateral subcapsular orchiectomy was done in all the patients. Estimation of serum prostate specific antigen (PSA) and prostatic acid phophatase (PAP) were done before, one month and then six months after the bilateral subcapsular orchiectomy. One month after the bilateral orchiectomy, 63% patients had more than 70% decrease in serum PSA values than the pretreatment values, while 47% patients had more than 70% decrease in the serum PAP values after one month of orchiectomy than the pretreatment values. Statistically significant decrease in serum PSA level was noted when pretreatment level of serum PSA was compared with serum PSA levels one month and six months after treatment. No significant difference was seen one month and six months after treatment when serum PSA values were compared with each other. Consequently, serum PSA estimation is the simple, cheaper, noninvasive and more sensitive than any other tumour marker for monitoring the response of treatment in patients with stage D prostatic carcinoma. It can better differentiate patients with stage D prostatic cancer showing good and poor response to the hormonal therapy.
