Effect of topical Bupivacaine on postoperative pain after tonsillectomy.

OBJECTIVE: To assess the effect of topical bupivacaine on post-tonsillectomy pain. STUDY DESIGN: Double blind, interventional experimental study. PLACE AND DURATION OF STUDY: This study was conducted at the Department of ENT and Head and Neck Surgery, Saidu Group of Teaching Hospitals, Saidu Sharif Swat over a period of six months from 1st August, 2003 to 31st January, 2004. PATIENTS AND METHODS: A total of 110 patients admitted for tonsillectomy were included in the study. They were divided into two groups. Group A received 0.5% bupivacaine soaked swabs packed in their tonsillar fossae while group B received normal saline soaked swabs in their tonsillar fossae after tonsillectomy. For data collection,pre-designed printed proforma was used for each patient. Pain scores based on visual analogue score were recorded at 3,5 and 8 hours post-operatively. Analysis of the results was done by minitab software and SPSS statistiscal package. RESULTS: Groups A included 55 patients of 24 males and 31 females between the ages of 6 and 30 years (mean age= 15.027 years, S.D=6.079). Group B included 55 patients of 31 males and 24 females between the ages of 7 and 29 years (mean age= 15.645 years, S.D=7.066).The main outcome measure was severity of pain by using visual analogue score. The mean postoperative pain scores for group A after 3, 5 and 8 hours were 4.836 (S.D=0.903), 3.9091 (S.D=0.5365),and 3.00 (S.D=0.3849) respectively. The mean post-operative pain scores for group B at 3, 5 and 8 hours were 6.182 (S.D=0.778), 4.9182 (S.D=0.6856),and 3.6727 (S.D=0.5949) respectively. CONCLUSION: Topical bupivacaine has a role in reducing post-tonsillectomy pain and facilitates early eating and drinking during the post-operative period.
