Endoscopic findings in Helicobacter pylori positive and negative patients with histologic gastritis.

OBJECTIVE: To determine the frequency of additional endoscopic lesions in oesophagus and duodenum in H.pylori positive and negative patients with histologic gastritis. DESIGN: Descriptive study. PLACE AND DURATION OF STUDY: Medical Unit- I Civil Hospital Karachi, from Jan 2004 to Oct 2004. SUBJECTS AND METHODS: A total of 306 patients having gastritis on endoscopy and confirmed by histopathology were included. Antral biopsies were stained for presence or absence of H.pylori and the frequency of associated lesions in oesophagus and duodenum were compared in both groups. RESULTS: Out of 306 patients, 47 % (n=144) were found to be H.pylori positive and 53 % (n=162) were H-pylori negative. The group of H.pylori +ve patients had isolated gastritis in 39.5%, gastroesophageal lesions in 22.9%, gastroduodenal lesions in 18.75% patients, and combination of lesions in oesophagus, stomach and duodenum in 18.75%. The group of H. pylori –ye patients had isolated gastritis in 42%, gastroesophageal lesions in 2I%, gastroduodenal lesions in 19 % and combinations of lesions in oesophagus, stomach and duodenum in 17.90%. The results showed that the presence of H.pylori was not associated with increased frequency of any specific endoscopic lesion. CONCLUSION: The frequency of H.pylori +ve and H.pylori -ye gastritis is about equal. There are no specific endoscopic lesions associated with H.pylori infection. So upper GI endoscopy cannot identify the presence of H.pylori, unless confirmed by histopathology or by other available methods before treatment.
