Outcome of ovulation induction in Polycystic Ovarian Syndrome.

OBJECTIVE: To assess the efficiency of different regimens for ovulation induction in poly-cystic ovarian syndrome (PCOS). DESIGN: Prospective cohort study. PLACE AND DURATION: Dept of obstetrics & Gynecology unit II CHK from march 2000 to march 2002. PATIENTS And METHODS: The study population comprised 60 Pcos patients evaluated for different regimens of ovulation induction. Clomiphene citrate was used as first line therapy. ln clomiphene resistant patients Gonadotrophins and laparoscopic ovarian diathermy (LOD) was compared. RESULTS: 60 patients were evaluated in this study clomiphene citrate was used as first line therapy in 30 patients. ln this group ovulation was successfully achieved in 25 (83.3%) while 18 patients (56.61) conceived with doses of 50-150 mg clomiphene per day for 5 days. Thirty patients with history of clomiphene resistance were divided into two groups. Half ( 15) of them received parenteral gonadotrophins while other fifteen had LOD. Induction of ovulation was successful in 9(60%) patients of gonadotrophins group and 12(80%) in LOD group. In both these subsets pregnancy rate was comparable with 6(40%) patients in each group who achieved conception. CONCLUSION: Clomiphene citrate is an effective first line therapy without major side effects. Results of gonadotrophins and LOD were equivocal in clomiphene resistant patients.
