Massive hemoptysis caused by pulmonary Aspergilloma.

Shakeel Ahmed, Age 26 years, Male was admitted in PAF Hospital Masroor on June, 2002, with complaints of low grade fever, cough with Hemoptysis and loss of weight, No past history of any lung disease. On general physical examination patient was thin built and anxious looking, his body temperature was 99°F, respiratory rate 22 per minute, blood pressure within normal limits, No other significant physical signs were noted. On inspection of the patient no significant changes were noted. On palpation mass, liver and spleen were not palpable. On auscultation bronchial breathing and crepitations were audible at right lower chest anterolaterally. On lab investigation, Blood complete picture was done, Hb%age 12.1 gm /dl, TLC 8.3 x 10e9 Ltr, Neutrophil 78%, Lymphocytes 16%, Eosinophil 4%, Monocytes 2% and ESR 34 mm fall at first hour end, LFT, Blood urea, Electrolytes and blood sugar were with in normal limits. Montoux test and Sputum AFB were negative. The culture of Sputum for Aspergillous fumigatus was not tested nor Antigen OR Antibodies were checked.
