Malignant transformation of Ameloblastoma — an unusual presentation.

A 55 years old male presented with history of swelling on left side of face of two months duration. The swelling had enlarged rapidly over a period of 15 days reaching its current size. It was associated with history of blood stained discharge from oral cavity of two months duration. Mouth opening was also restricted. Patient did not complain of headache, earache, visual loss or any swelling in the neck, but his major complaint was disfiguring huge swelling on left side of face. On checking previous record it was discovered that patient had presented with lump on same side of face which was diagnosed on biopsy as ameloblastoma for which partial maxillectomy was carried out 4 years back. Examination of lump revealed firm, non tender, afebrile, 6 x 6 cm swelling on left side of face extending from zygoma to left lateral nasal wall and from lower eyelid to lower border of masseter producing the significant deformity. It was pushing the eyeball forewards with reduced eye opening although eye movements were normal and vision was intact. The lump was fixed to underlying structures almost infiltrating the overlying skin but skin was mobile. Facial nerve was intact. Mouth opening was restricted. To confirm diagnosis, FNAC of the mass from outside and intraoral biopsy was done. Both showed ameloblastoma. Because of very aggressive picture, radical excision was planned. Total maxillectomy via weber-fergussion approach was carried out.
