Gun shot trauma of the maxillofacial region: an analysis of 66 cases.

Objective: To study the epidemiology and outcome of cases of gunshot trauma of maxillofacial region. Design & Duration: Descriptive, analytical study conducted over a three years period. Setting: Dept. of Maxillofacial Surgery, Mayo Hospital, Lahore. Patients: All patients presenting with gunshot trauma of the maxillofacial region. Methodology: The data of all the patients presenting with gunshot trauma of the maxillofacial region was analyzed. Results: A total of 66 cases of gunshot trauma of maxillofacial region were treated during the three years study period, including 59 males and seven females. Second, third and fourth decades of life were equally involved in this trauma. Lower half of the face was involved in 47 cases and upper half in 14 cases, while five cases had involvement of both the halves. Tracheostomy was done in 11 cases. Fracture reduction and intermaxillary fixation was done in 49 cases, rigid fixation with plate and interosseous wiring in 10 cases, while the seven remaining cases were dealt with conservatively. Most of the soft tissue defects were dealt with conservatively, but 21 cases needed different flaps for reconstruction of these defects. Pectoralis major was found the most useful option and was utilized in five cases. Conclusion: Gunshot wounds of the maxillofacial region are not uncommon. They may end up in extensive soft tissue loss and may need various local and regional flaps for tissue bulk, skin cover and oral lining.
