PakMediNet Discussion Forum : Medicine : What will be diagnosis?
21 year presented unconscious wid 7 day H/O high grade fever and 2 days drowsiness. Examination: Pulse 120/min, B.P 100/70, temp 103 F, Jaundice +, Coma grade IV, Hb 8.9g/dl, Bilirubin 2.9mg/dl, SGPT 65iu, Alk phosphatase 102 iu, CXR , ECG and RPM and electrolytes are normal. If differential is possible then uptill 3 is workable. thank u.

I would think of:
Cerebral Malaria
Leptospirosis
Acute Hepatic Failure with Encephalopathy
Bacterial Meningitis with Sepsis and Hypotension

Bilirubin + raised liver enzymes means hepatitis which can be due to infection, sepsis or hypotension (shock liver). All these conditions can lead to liver failure and Encephalopathy.
