Metal Cap Guided Circumcision Technique: Comparison with Forceps Guided and Plastibell Techniques.

Objective: To compare the results of metal cap device, plastibell and forceps guided / guillotine technique for circumcision. Place and Duration: The study was conducted on 150 patients, at the Department of General Surgery, D.H.Q. Teaching Hospital, Rawalpindi, from January 2002 to June 2002. Study Design: It was a hospital based comparative study. Patients and Methods: One hundred and fifty healthy male children, newborn to ten years of age, were randomly distributed in three equal groups, for Guillotine technique, Plastibell and Metal cap device. The main outcome measures studied were apprehension of damaging glans penis, simplicity of technique, effective hemostasis, cosmetic appearance, post operative care, cost effectiveness, effect of age/size and any complication. The circumcised penis was examined after 24 hrs, 1 week and 1 month. Results: Although apprehension of damaging glans penis was 64%, with guillotine method, 60% (30/50) of surgeons found it a simpler method. However, intra-operative homeostasis, post-operative care, good cosmetic results and cost effectiveness were better achieved with metal cap device as compared to rest of the two methods. Conclusion: Metal cap device produced better results than the Guillotine method of circumcision. It was not associated with complications inherent to plastibell device. It was a simple modification of the guillotine method, and can be used by surgeons at different level of experience without endangering the glans.
